Catherine Klostermeier, Geriatric Care Manager, is the founder of Aging Transitions, and is a member of the National Association of Professional Geriatric Care Managers. Catherine experienced the overwhelming maze of elder care when her mother in-law was diagnosed with a Parkinson's type disease. She became a Professional Geriatric Care Manager to guide families through the multiple decisions regarding their older loved one who needs help.
Catherine provides guidance to family members and friends and develops an action plan when the changes in the life of an older loved one are more than the current caregiver can manage alone.
For example, the plan might be to hire a home care aid for 4 hours per day, install grab bars, get a night light for the bathroom, schedule a nurse to set up the medications every two weeks, schedule doctor appointments, and accompany the older adult to the appointments to make sure the right questions are asked and the answers written down. In this plan, Catherine would recommend home care companies, handymen, and nurses for the family or friends to choose from.
Catherine understands how difficult it can be for family and friends when older loved ones begin to decline physically or cognitively. No one wants the changes. Everyone wants things to go back to how they were when the older loved one was active and well. It’s a difficult road and you don’t have to travel it alone.
